Checks if the trimmed first line is 'Approved.' or 'Rejected.' (matching VS-UX-2's
submitPlanReview leading prefix).
When the first line does not decide, inspects @form: / @{"_mode":"form",...} tokens via
ConversationUtility.ParseSpecialContent(text) for a decision field ('approve' or 'reject').
Otherwise strips special-content tokens and an optional leading 'Approved.' prefix, returns
null on empty text, 'Approve' when the trimmed remainder starts with 'approve'
(case-insensitive), and 'Reject' otherwise.
