Rewrites the question name in every decisions.<step>.<question> reference to one step.
RewriteDecisionReferences renames a step; this renames one of its questions, in the same
grammar and keeping each reference's form the same way: .intent stays a dot when the new name
can follow one, and becomes ['<name>'] when it cannot. References to other steps, malformed uses
and text inside string literals are left as written.
