Statically scans an ordered list of SQL migrations for FOREIGN KEY ([Col]) declarations
(in both CREATE TABLE and ALTER TABLE) and verifies that each FK column is covered by
either its IDX_AUTO_MJ_FKEY_{Table}_{Column} index or an index on Table whose leading
key column is Column (when allowLeadingColumnIndex is true; defaults to false so
IDX_AUTO_MJ_FKEY_* naming is enforced by default to match MC4).
