Answers one Decision node's questions, in one call.
A seam like the other runners, so the dispatcher's Decision handling is testable without a model.
Unlike them it has a default — AIDecisionTaskRunner — because this package can make the call
itself: it already depends on @memberjunction/ai-prompts, and a Decision needs nothing a host
owns. A host with its own decision strategy supplies one.
MUST make exactly one decision call per invocation. The questions a fork needs are asked
together on purpose; splitting them multiplies cost for no gain.
